Telephonic Behavioral Health Care Manager
About the role
The Care Manager, Telephonic Behavioral Health 2, works in a telephonic environment to assess and evaluate members' needs and requirements to achieve and/or maintain optimal wellness state. They guide members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members.
Responsibilities
- Affirmatively interpret and independently determine appropriate courses of action
- Employ a variety of strategies, approaches, and techniques to manage physical, environmental, and psycho-social health issues
- Identify and resolve barriers that hinder effective care
- Monitor patient care through assessments and/or evaluations to ensure the patient is progressing towards desired outcomes
- Create member care plans
- Understand department, segment, and organizational strategy and operating objectives
Requirements
- Current valid social service license, e.g. LMSW, LCSW, LPC, LMHC
- Master's degree in a behavioral-health field, such as social work, counseling, or related health discipline from an accredited university
- 2+ years or more of experience working as a behavioral health professional
- Experience with behavioral change, health promotion, and coaching
- Proficiency with Microsoft Office Programs, e.g. Word, Excel & PowerPoint
Preferred Qualifications
- Experience supporting patients telephonically
- Experience working in Community Mental Health or as part of a crisis response team or Assertive Community Treatment (ACT) team, and substance use disorder treatment
- Experience working with both children and adults
- Bilingual (English/Spanish)

Schedule
Eight (8) hour shift between 8:00 AM and 6:00 PM in your local time zone. Occasional over-time may be requested as business needs.
Pay Range
$59,300 - $80,900 per year
