Role Overview

A Telemedicine Medical Oncology Nurse Practitioner (NP) delivering comprehensive survivorship care through a telehealth-first model. This remote position emphasizes longitudinal patient relationships rather than episodic visits, focusing on managing long-term and late effects of cancer treatment, coordinating surveillance, and providing education, counseling, and psychosocial support.

Key Responsibilities

• Conduct virtual survivorship visits and develop personalized care plans
• Manage chronic conditions related to cancer treatment
• Coordinate evidence-based surveillance and recurrence monitoring
• Collaborate with multidisciplinary teams including oncology, behavioral health, and care navigation
• Contribute to clinical workflow, SOPs, and care protocol development
• Engage in quality improvement and program scalability efforts

Qualifications & Skills

• At least 3 years of experience as an Oncology and Survivorship Nurse Practitioner
• Strong clinical judgment and comfort working virtually
• Excellent patient-centered, empathetic communication skills
• Ability to thrive in a fast-paced, evolving startup environment
• Highly organized with keen attention to detail
