Responsibilities
- Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into the ambulatory care setting.
- Critically assesses patient data to proactively mitigate impending changes in condition and establishes consults to interprofessional team as appropriate.
- Learns and implements complex individual plans of care with defined goals in collaboration with other members of the interprofessional team and the patient, family, or caregiver.
- Utilizes and champions scientific inquiry for evidence-based practices, such as participating in journal clubs, facilitating practice change based on new knowledge, recommending new standards of care, conducting literature searches, and disseminating new knowledge.
- Advances nursing practice through involvement in community service events, patient/family education groups, advisory boards, ethics committees, and teaching roles.
- Analyzes gaps in practice standards and provides feedback to peers in collaboration with leadership to enhance safe patient care.
- Evaluates the patient and family experience, and adjusts care provided to improve patient satisfaction and quality of care.
Requirements
This is a safety-sensitive position. Essential functions include working in a constant state of alertness and in a safe manner.
Qualifications
- Education: Nursing Diploma/Associate's degree preferred, Bachelor's degree in Nursing preferred.
- Experience: 2-5 years of nursing experience preferred, 5-10 years of supervisory experience preferred.
- Licenses & Certifications: Current Registered Nurse (RN) license required.
Skills
Knowledge of evidence-based practices, critical thinking, problem-solving skills, and strong communication and interpersonal skills are essential.

Pay
Pay is aligned with the new job description and includes a sign-on bonus eligible up to $15,000 for external candidates.
Schedule
Shifts may vary, including nights 7 pm to 7:30 am every 3rd weekend.
