Team Counselor - Mobile

About the Role

Integrated Team Counselors (ITCs) provide clinical interventions and peer and family support to facilitate engagement, support functioning, and maximize symptom stabilization and self-management of individuals. ITCs assist individuals in developing skills, establishing natural supports, and resources to live successfully in the community. Working as part of the ACCS team under the guidance of the Team Leader, staff will provide services and interventions identified in the Treatment Plan, including education, supervision, and direct supports.

Responsibilities

• Provide input to person-centered assessment and service planning, advocacy, coordination, and linkage with other services and the community.
• Provide rehabilitation interventions, including skills training in community environments.
• Offer education, training, and physical intervention when needed to assist persons served in learning life skills such as budgeting, banking, transportation, housekeeping, personal hygiene, self-medication, nutrition, and health.
• Model and teach skills for development of positive relationships and negotiation of conflict.
• Assist persons served with accessing necessary and appropriate health care.
• Maintain regular daily, monthly, and annual clinical records in keeping with DMH, agency, and division policies and procedures, and within required timelines.
• Consistently perform all medication administration and documentation procedures in accordance with Riverside and State policies and regulations relating to the Medication Administration Program (MAP).

Requirements

• Demonstrated ability to work effectively in a team environment.
• Excellent written and verbal communication skills (in the English language).
• Valid driver's license and available, dependable transportation.

Qualifications

• Bachelor's Degree preferred or High School diploma required.

Schedule

40 hours per week:

• Tuesday: 1 p.m. - 9 p.m.
• Wednesday: 10 a.m. - 6 p.m.
• Thursday: 8 a.m. - 4 p.m.
• Friday: 1 p.m. - 9 p.m.
• Saturday: 10 a.m. - 6 p.m.

Pay

$21.37/hour
