About the role
The TAY Housing Navigator provides housing location, navigation, and stabilization services to youth exiting the County of Los Angeles Child Welfare System (18-24 years old) who have been identified and referred by the Department of Children and Family Services (DCFS).
Responsibilities
- Affirmatively assess needs and stabilize youth exiting care to appropriate housing types.
- Partner with youth to develop and implement Individual Action Plans (IAPs).
- Support youth in accessing services to achieve their IAP goals, including mental/physical health services, mainstream benefits, and education.
- Provide housing stabilization services, such as money management, life skills, and self-advocacy.
- Interface with local landlords and property managers to identify and secure housing options.
- Participate in care coordination meetings with DCFS and other relevant agencies.
- Enter data in required databases and compile monthly service reports.
- Attend partner meetings and staff development training.
- Data collection/entry into HMIS & Salesforce databases for reporting and evaluation.
- Maintain accurate client case files according to CRCD protocols.
- Conduct weekly face-to-face contacts with clients.
- Act as a liaison among various stakeholders to ensure housing stability.
- Coordinate with CRCD’s Workforce Development Department to provide career pathways training and employment opportunities.
Requirements
- 1-2 years’ experience or related work and a Bachelors-level Degree; or 5+ years’ experience with an Associates-level Degree.
- Experience working with youth experiencing homelessness and housing instability, and/or experience in foster care, probation, mental health systems.
- Reliable, flexible team player with a good work ethic and ability to set and maintain personal boundaries.
- Demonstrated ability to work on multiple projects simultaneously and meet deadlines.
- Knowledge of South Los Angeles community and social services resources.
- Background in Mental Health services, Trauma Informed Care, and/or Harm Reduction.
- Proficient in Microsoft Office Suite.

Pay
$50,000-$55,000 annually
