About the role

As a Targeted Case Manager, you will support people with developmental disabilities in conjunction with their chosen support network to identify, select, obtain, and utilize paid services and natural supports. Your work will enhance the independence, productivity, and integration of those we serve, aligning with their lifestyle preferences and needs.

This is a hybrid position, blending work from a home office and community-based activities.

Responsibilities

  • Manage development, updating, and review of the Person-Centered Support plan using the approved template in participation with the person and their support network.
  • Monitor the quality of services delivered to individuals served.
  • Document individual progress and maintain records of persons served.
  • Conduct and record daily billable activities.
  • Provide assistance and information to individuals regarding types and availability of community services and support options.
  • Access natural supports and generic community support systems, including pursuing means for gaining access to needed services and entitlements.
  • Modify service systems when necessary to increase accessibility if needed by the person.
  • Perform monitoring and follow-up to ensure the person-centered support plan and related services are effectively implemented and address the person’s needs.
  • Assist with transition and portability, including planning and arranging for services to follow the person when they move.
  • Comply with KDADS Rules of Conduct for Case Managers Serving People With Developmental Disabilities.

Qualifications

  • A minimum of six months of full-time experience in the field of human services.
  • Either a bachelor’s degree or additional full-time experience in the field of developmental disabilities services, which may substitute for the degree at the rate of six months of full-time experience for each missing semester of college.

Schedule

Generally 9 a.m.–5 p.m., Monday–Friday. On rare occasions, you may need to accommodate a family’s schedule outside of these hours.
