Position Summary
Under the direction of the Director of Supports Coordination Services, the Supports Coordinator is responsible for:
- Providing comprehensive case management services
- Developing and maintaining Individual Support Plans (ISPs) using a person-centered planning approach
- Making sure services are delivered as written in the ISP
- Conducting required face-to-face visits to monitor health and welfare
- Coordinating direct services and community resources
- Responding to emergency situations to ensure participant safety
- Facilitating annual ISP reviews and reassessments
- Maintaining documentation in HCSIS
- Supporting individuals and families throughout the planning and service process
- Providing information on fair hearing rights and assisting with requests
- Participating in community outreach and agency fundraising initiatives
- Completing all required trainings
Key Responsibilities
- Conduct interviews and gather medical, educational, social, and vocational information
- Develop, implement, monitor, and update Individual Support Plans (ISPs)
- Ensure services are delivered as written in the ISP
- Conduct required face-to-face visits to monitor health and welfare
- Coordinate direct services and community resources
- Respond to emergency situations to ensure participant safety
- Facilitate annual ISP reviews and reassessments
- Maintain documentation in HCSIS
- Support individuals and families throughout the planning and service process
- Provide information on fair hearing rights and assist with requests
- Participate in community outreach and agency fundraising initiatives
- Complete all required trainings
Requirements
- Current FBI, Criminal History, and Child Abuse Clearances (required at hire and maintained per regulations)
- Valid Pennsylvania Driver’s License
- Current automobile insurance (recommended coverage: $100,000/$300,000 liability and $100,000 property damage)
- 3-year driving record check (updated annually)
- Willingness and ability to travel throughout service counties
Physical Requirements
- Frequent verbal and written communication
- Prolonged sitting and standing
- Ability to observe, assess, and document
- Use of computer and basic math skills

Qualifications
- Required: Bachelor’s degree from an accredited college or university
- Minimum of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or related social sciences
- Preferred: Experience working with individuals with intellectual and developmental disabilities
- Knowledge of Pennsylvania MH/ID systems, special education systems, and public benefits (Social Security, welfare, etc.)
