Supported Living Specialist

About the role

Chimes is a not‑for‑profit organization that assists people with intellectual and behavioral challenges to achieve their fullest potential through a comprehensive range of services—including educational, employment, vocational, residential, rehabilitative, and behavioral health—delivered via a network of national and international affiliates. The Supported Living Specialist will help each consumer reach a maximum level of self‑sufficiency through individualized interventions and support within a therapeutic milieu, following service plans developed collaboratively with consumers.

Responsibilities

  • Provide services as defined by the program description, best practice standards, licensure standards, and Recovery Model Principles.
  • Assist the program coordinator with assigned administrative duties.
  • Monitor effectiveness of service delivery using outcome measures such as the Quality of Life Inventory, consumer satisfaction surveys, and treatment/service reviews; adjust delivery as needed to improve outcomes.
  • Deliver services dictated by client and program needs.
  • Develop and implement service plans per the program description.
  • Develop and maintain linkages with adjunct providers to coordinate consumer services.
  • Identify accessible, beneficial resources for clients and encourage their utilization.
  • Intervene appropriately in crisis situations.
  • Provide instruction and hands‑on support to promote skill development identified in service plans.
  • Support consumers in maintaining healthy relationships and resolving conflicts.
  • Maintain accurate records of billable services and submit them promptly as required.
  • Complete other responsibilities assigned by the direct supervisor.

Requirements

  • Associate’s Degree.
  • At least 2 years of mental‑health direct service and/or case‑management experience (additional related experience or education may be considered in lieu of the degree/experience requirement).
  • CPRP certification is a plus.
  • Clearances: criminal background check and fingerprinting, federal criminal history check, valid driver’s license, and verification of no Medicaid/Medicare exclusion.

Pay

$20+ per hour, based on experience.

Schedule

Full‑time, Monday through Friday, 9:00 am – 5:00 pm (flexible scheduling).

Location

Exton, PA – Mobile Psychiatric Rehabilitation Program.
