Supervisor, RN, Part Time Days

Responsibilities

• Makes daily compliance rounds on units at least at the beginning and end of shift to observe, examine, or interview residents, verify information from the ongoing shift, monitor regulatory compliance, determine staff assignments, and provide/receive reports.
• Provides orientation to unit and unit routines to new staff members or staff from temporary agencies.
• Describes care plan goals and approaches to direct care staff to ensure consistent implementation across all shifts by all caregivers.
• Seeks input from nurse aides about residents’ condition, functional abilities, preferences, and alternative approaches to care.
• Monitors delivery of care and services throughout the shift to ensure resident needs are met, tasks are completed, and the work of direct care staff meets quality and quantity standards.
• Administers medications, treatments, and provides direct care to residents on the unit according to physician orders and in compliance with facility policies and procedures.
• Performs physical assessments (or gathers appropriate data) of new admissions and current residents as indicated by changes in condition or regulatory requirements.
• Completes assigned sections of resident assessment instruments used in the facility.
• Develops, implements, evaluates, and revises plans of care with measurable objectives and timetables, in collaboration with the interdisciplinary team and resident, to assist each resident in attaining or maintaining the highest practicable physical, mental, and psycho-social well-being.
• Coordinates care and delivery of services with all disciplines.
• Consults with and refers relevant resident care issues to interdisciplinary care team members.
• Modifies approaches in plans of care to accommodate resident needs and preferences, manage risk factors for functional decline, and improve functional abilities.
• Trains and develops staff by conducting skills observations of nurse aides, identifying learning needs, and providing on-unit teaching opportunities.
• Reports information to DON Unit Managers as part of staff performance evaluations.
• Completes required documentation of care and services delivered during the shift, including subjective findings, objective symptoms, interventions, and resident responses to interventions.
• Completes documentation of special circumstances, such as accident/incident reports, in compliance with state and federal laws and regulations.
• Completes the written 24-hour facility report and participates in verbal shift reports, noting significant changes in condition, admissions, transfers, discharges, use of restraints, incidents, medication errors, and resident or family dissatisfaction.
• Immediately informs the resident, consults with the resident’s physician, and notifies the designated family member and/or legal representative for significant changes, accidents, or decisions to alter treatment, transfer, or discharge the resident.
• Promptly notifies the resident, designated family member, and/or legal representative of changes in room or roommate.
• Reports relevant information to the DON and Unit Manager, including interdisciplinary team functioning, regulatory compliance issues, resident risk factors, changes in behavior, family dissatisfaction, treatment refusals, unexplained injuries, staff behavior, staffing needs, and equipment or supply needs.
• Monitors staff for compliance with OSHA mandates on workplace safety, including hazard communication and bloodborne pathogens.
• Provides immediate first aid in cases of workplace exposure and refers employees for post-exposure follow-up.
• Takes appropriate job actions with staff members, applies facility policies on progressive discipline, and removes staff from the unit if necessary to avoid endangerment to residents or other staff.
• Assists the ADON with preparation for long-term care surveys, attends survey training, and interacts with state surveyors as instructed by a supervisor.

Requirements

• Graduate of an accredited school of nursing.
• Current, active license as a Registered Nurse in Pennsylvania.
• One year of experience providing direct care to residents in a long-term care setting.
• Prior experience as a Charge Nurse or similar supervisory role preferred.
