Supervisor Care Management

About the role

This position is responsible to the Manager of Care Management. This role oversees the day-to-day operations of Care Management, including staffing scheduling, supervising and mentoring staff, ensuring standardization of care management, assisting in responding to customer requests and problems, providing leadership and monitoring for compliance and performance, and performance improvement.

Responsibilities

• Participates as a member of the interdisciplinary team to ensure that selected clinical quality criteria are met.
• Achieves a target query rate of 12% and provides supervision to social work staff, ensuring all rules and regulations are followed.
• Manages and tracks CDMP and win worksheets to maintain their professional appearance and quality.
• Ensures the accuracy and timeliness of interdisciplinary team rounding and MDR rounds to meet patient needs.
• Assists team members in adjusting schedules as needed to ensure adequate coverage and addresses any issues or concerns related to workflow or patient discharge disposition.

Requirements

• Bachelor's Degree Equivalent experience accepted in lieu of degree.
• Related certification: Registered Nurse, Current Ohio licensure.
• 3-4 years experience in a related field.
