About the role
Supervises daily operations of discharge planning and social services in the inpatient, observation, and emergency department, serving as the primary point of contact for inpatient unit Care Management barriers and concerns.
Responsibilities
- Implements corporate Care Management strategies in collaboration with facility management.
- Actively participates in multidisciplinary rounds, ensuring active staff participation and effective discharge planning and barrier resolutions.
- Provides discharge planning and progression of care expertise to the interdisciplinary team.
- Leverages reports and dashboards to monitor the accuracy of care manager team's documentation and compliance with regulations.
- Facilitates care conferences for complex transitions and placements as needed.
- Participates in hospital and medical staff meetings to review patients with high dollar, extended length of stay, or difficult placement.
- Reports pertinent quality and risk issues to appropriate individuals, departments, and committees in a timely manner.
- Adds addresses patient, staff, physician, and department complaints and follows up with timely communications and escalations.
- Participates in hiring, onboarding, educating, evaluating, and mentoring staff.
- Performs other duties as assigned.
Requirements
- Knowledge of medical necessity for appropriate patient status and level of care
- Knowledge of IMM, MOON, Advanced Directives, Multidisciplinary Rounds, Social Services, Value Based Programs, and other Care Management compliance guidelines
- Ability to lead others effectively while maintaining appropriate compliance
- Knowledge of Centers for Medicare & Medicaid Services (CMS) Conditions of Participation for discharge planning
- Ability to manage multiple tasks and prioritize level of importance
- Critical thinking skills
- Customer service skills
- Ability to implement process improvement processes
- Experience with PC applications and development of reports
- Ability to negotiate optimal resolution of issues
- Ability to manage multiple tasks and prioritize level of importance
- Ability to engage in complex clinical decision-making
- Strong professional verbal and electronic communication
- Ability to analyze, evaluate, and monitor data
- Ability to foster positive Interdisciplinary Team relations and encourage employee engagement
- Ability to work independently and/or as part of a team
Qualifications
- Bachelor's degree in social work
- 2+ years of social work experience
