SUDP/SUDPT Assessor

About the role

This dynamic position plays an important role in helping Evergreen Treatment Services (ETS) accomplish our mission. The Substance Use Disorder Professional (SUDP) is part of the Behavioral Health and Counseling team, serving as the primary care team coordinator for assigned patients. This role also works in connection with King County Public Health Pathways Clinic to conduct assessments for patients requiring SUD services. The position supports community partner organizations by providing onsite assessment services where they are not currently offered. Additionally, the role provides individual and group counseling, case management, and coordinates overall patient care while patients are enrolled at ETS.

Responsibilities

• Perform Substance Use Disorder assessments in office-based, inpatient, or community settings as necessary; develop treatment plans.
• Respond to referrals from various community locations to perform assessments, make treatment recommendations, and coordinate care efficiently and safely.
• Develop relationships with community resources and create treatment and safety plans to assist individuals and their natural and professional supports.
• Provide and connect patients and professionals with accurate program and agency resources.
• Implement established agency policies and treatment procedures for assigned patients, including initial assessment, individual and group counseling, and case management services.
• Maintain professional boundaries with patients and coworkers.
• Complete prompt and accurate progress notations of all interactions with patients in assigned patient files.
• Prepare and submit the following documents professionally and promptly:
  • Initial assessment and treatment plan
  • 90-day treatment plan
  • Responses to positive urinalysis/breathalyzer results
  • Administrative Taper notices
  • Letters of assistance and referral for assigned patients
  • Closing treatment summaries for discharged patients
  • Daily activity forms
  • Required state and agency recording forms (such as TARGET reports)
• Protect the confidentiality of all patients and the integrity of professional staff communication and behavior, adhering to 42 CFR, Part 2 federal regulations.
• Serve as a leader or co-leader of therapy or educational groups and provide other treatment activities.
• Meet with primary supervisor for clinical consultation, skill development, and review of patient documentation and treatment planning.
• Coordinate assigned patient treatment with other staff departments (e.g., dispensary, reception, medical, and psychiatric staff).
• Participate in Treatment Team meetings to review patient treatment progress, address appeals of administrative actions, and resolve program non-compliance issues.
• Some local travel may be required on behalf of the agency.

Requirements

• Credential: SUDP as defined in WAC 246-811-030.
• Education and/or Relevant and Lived Experience:
  • Academic training in the social service field and in substance use disorder treatment, in accordance with Department of Health qualifications for a licensed Substance Use Disorder Professional/Trainee (SUDP/T).
  • Work experience in the social service field and in substance use disorder treatment, as stipulated in the WAC for SUDP/T.
  • Experience in substance abuse treatment planning and counseling.

• Counselors with a history of personal substance abuse must have been abstinent for a minimum of three years prior to employment.

• Excellent organizational skills and ability to prioritize workload, work independently, and complete tasks in a timely manner.
• Dependable, able to work under pressure, receptive to change, and cooperative in problem-solving.
• Flexible team player with excellent attention to detail.
• Ability to maintain confidentiality and use discretion when handling highly sensitive information.
• Ability to set boundaries, resolve conflict, and de-escalate issues.
• Computer literate, with basic knowledge of Microsoft Office Suite and initiative to stay current with technological changes.

• Physical Conditions and Requirements:
  • Potential exposure to illicit drug residues, fumes, or other bio-hazardous materials; training will be provided to limit risks.
  • Potential exposure to bloodborne pathogens; policies and procedures are in place to address this risk.
  • Regularly required to sit, use hands, reach with hands and arms, talk, and hear.
  • Frequently required to stand, walk, and kneel; occasionally to climb, balance, or stoop; rarely to crouch or crawl.
  • Must occasionally lift and/or move up to 30 pounds.
  • Requires close, color, and peripheral vision, and the ability to adjust focus.
  • Moderate noise level in the work environment.
• Equipment Used:
  • Computer, photocopier, fax machine, and phone.
  • Possible use of a program vehicle, requiring a valid Driver’s License and acceptable driving record.
