Case Manager Requirements
- College degree from accredited university, or equivalent work experience
- 2+ years of experience working in the behavioral health field
- Bachelor-level SUD certification (LADC equivalent) or CRSW/CADC
- Valid Drivers license required
Case Manager Expectations
- Conduct a case management interview with patient to determine needs
- Obtain FMLA, Short Term Disability Insurance, and pending legal issues
- Produce necessary documentation for employer, schools, legal, etc.
- Meet with patients weekly for case management appointments
- Develop and maintain strong relationships with patients and families
- Maintain ongoing communication with patients' families
- Develop and implement aftercare plans for all patients
- Ensure patients follow established aftercare plans
- Work with outreach team or company representative to determine aftercare plan
- Update and modify patient discharge dates based on Utilization Review, Clinical, and Medical team
- Accurately share information in team meetings
- Ensure medical records are sent to the patient's new facility
- Facilitate SUD and Mental Health groups
- Maintain a basic understanding of health insurance benefits and terms
- Remind patients of program rules and regulations and report infractions appropriately
- Interact with patients respectfully while maintaining professional boundaries
- Protect patient information in accordance with organizational policies and laws
