Case Manager Substance Abuse

About the role

The substance use case manager will provide comprehensive case management services to individuals with substance use disorders. This position is for the evening expanded hours program, evenings are required. This role will focus on assessing client needs, developing individualized service plans, coordinating treatment, and support services. This will help promote long-term recovery. The case manager will work collaboratively with the senior counselor, client, families, treatment providers, and community resources to ensure continuity of care and support harm reduction, recovery oriented goals. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.

Qualifications

  • Educational Requirement: Bachelor's degree in related field required.
  • Licenses/Certifications: CADC preferred.
  • Driver’s License: Required.
  • Training: Annual, American Heart Association BLS and Heart Saver First Aid required within 90 days of hire or transfer. CPI required within 90 days of hire or transfer.
  • Background Check: Ability to pass a background check through fingerprint process and drug screen as required.

Duties and responsibilities

  • Conduct initial screenings and ongoing needs assessments.
  • Develop, implement, and monitor individualized treatment and recovery service plans.
  • Provide crisis intervention, individual counseling, and strength-based support.
  • Facilitate referrals for mental health, housing, employment, court, and other supportive services.
  • Maintain accurate, timely, confidential documentation in compliance with HIPAA state regulations and AtlantiCare addiction services policies.
  • Ensure compliance with grant requirements, program standards, and funding.
  • Coordinate or provide transportation for clients when needed.
  • Monitor client progress, provide follow-up, and adjust service plans as necessary.

Work environment

This position requires the case manager to work in the following environments: office, community, and field-based work. May require occasional evening or weekend hours depending on program needs. This position requires repetitive motion, hearing, pushing, reaching, and reading, seeing, speaking, standing and walking a majority of the time. Spends time with such equipment as computer terminal, fax machine, copier, and telephone. Occasional driving also required.
