As a Case Manager you will play a direct role in saving lives by providing and promoting an environment of long‑term recovery, partnering with the Clinical Director and Executive Director to establish and maintain a therapeutic environment for our residents.

Responsibilities

  • Provide case management and care coordination services for patients enrolled in treatment.
  • Coordinate continuum of care activities for assigned patients, ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral source requirements.
  • Coordinate patient assessments, treatment planning, and crisis intervention services.
  • Provide training and facilitate linkages to community resources such as financial assistance, housing, food, clothing, medical, educational, vocational, and recreational services.
  • Assist patients in networking with family members, friends, and organizations.
  • Support patients in performing daily living activities and coordinate criminal‑justice services as needed.
  • Conduct individual and family educational sessions and coordinate treatment and discharge activities.
  • Timely coordination of issues or activities relevant to the treatment team and facility milieu.
  • Adhere to organizational policies, procedures, performance‑improvement initiatives, and confidentiality standards.
  • Attend team meetings and complete all required training courses.
  • Perform other duties as assigned.

Requirements

  • Bachelor’s degree in a Human Services field from an accredited college or university.
  • Valid/current driver’s license.
  • Ability to travel up to 15% as required.
  • Preferred: Previous experience in case management and/or utilization review.
