About the role

As a Case Manager, you play a direct role in saving the lives of our patients each day by providing and promoting an environment of long-term recovery. You will partner with the Clinical Director and Executive Director to establish and maintain a therapeutic environment and routine for our residents.

Responsibilities

  • Provide case management and care coordination services for patients enrolled in treatment.
  • Coordinate continuum of care activities for assigned patients, ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements.
  • Coordinate patient assessments, treatment planning, and crisis intervention services.
  • Provide training and facilitate linkages for the use of community resources.
  • Monitor service delivery and assist patients in obtaining necessary services (e.g., financial assistance, housing assistance, food, clothing, medical services, educational services, vocational services, recreational services, etc.).
  • Assist patients in networking with family members, friends, and/or organizations.
  • Assist patients in performing daily living activities.
  • Coordinate criminal justice services.
  • Conduct individual and family educational sessions.
  • Engage in various duties related to coordinating treatment and discharge activities, as needed.
  • Coordinate, in a timely manner, issues or activities relevant to the team and/or the team's relationship within the facility or treatment milieu.
  • Adhere to organizational policies, procedures, performance improvement initiatives, and maintain confidentiality.
  • Attend team meetings and complete all training courses timely as required.
  • Other duties as assigned.

Requirements

  • Bachelor’s degree in the Human Service field from an accredited college or university.
  • Valid/current Driver's License.
  • Ability to travel up to 15% may be required.

Qualifications (Preferred)

  • Previous experience in case management and/or utilization review.
