Case Manager (Substance Abuse Treatment Facility)

Responsibilities

• Completes Intake and Case Management Assessments.
• Covers inter-departmentally to facilitate clients attending approved medical appointments.
• Begins correspondence with legal contacts such as probation, courts, and judicial assistants to help clients make informed decisions about discharge planning.
• Attempts to prevent further legal stress or charges during treatment.
• Links clients with community resources based on clinical recommendations and approval.
• Coordinates with referral sources, PC/IOP programs, psychiatrists, and individual therapists to provide all necessary information.
• Attends regular clinical staff meetings.
• Demonstrates adaptability and flexibility to changes in the work environment.

Qualifications

• Versatile verbal and written communication skills.
• Ability to speak clearly and concisely over the telephone.
• Previous treatment experience required.
• Ability to work independently and adapt to a fast-changing environment.
• Ability to multitask in a high-pressure setting.
• Compassion towards clients.
• Strong ethical boundaries, when needed.
• Excellent time management skills.
• Familiarity with electronic medical records systems, KIPU preferred.
