Street Outreach Case Manager - 3rd Shift (Tues–Sat)

About the role

The position will be based in Mesa, Arizona and requires availability from Tuesday to Saturday, 3:00 PM to 11:00 PM. Additionally, there is a $3 shift differential.

Responsibilities

• Connect with and motivate people living on the streets towards self-sufficiency
• Aid homeless individuals in accessing social services and identifying resources to end their homelessness
• Create and implement individualized case plans for outreach prospects
• Maintain relationships with community stakeholders and attend relevant events
• Enter and maintain accurate client information in the PRM database
• Lead or participate in devotions, prayers, or other activities promoting a credible Christian witness and lifestyle

Requirements

• Bachelor’s degree in Social Work, Sociology, Psychology, Counseling, or a related field, or 4 years of experience in case management, social work, or a similar setting
• Certification in Peer Support, preferred
• Current Arizona driver’s license with a clean driving record for at least 2 years (verified through company MVR check)
• Knowledge of issues affecting the street homeless population and familiarity with resources to help people exit homelessness
• High level of composure and ability to remain calm in stressful situations
• Discretion and understanding of the importance of confidentiality regarding client information
• Excellent communication skills and proficiency in Microsoft Office applications
• A strong and credible Christian witness and lifestyle that aligns with the Mission's Statement of Faith
