Street Outreach Case Manager - 3rd Shift (Sun–Thurs)

This position is based in Mesa, Arizona and requires availability Sunday through Thursday, 3:00 PM to 11:00 PM, with a $3 shift differential. Reliable transportation is required.

Responsibilities

• Connect with and motivate people living on the streets to strive toward self-sufficiency.
• Assist homeless individuals in navigating the social service system to identify resources that help end their homelessness.
• Set goals and develop individualized case plans with each outreach prospect.
• Establish and maintain relationships with community stakeholders and attend community outreach events.
• Ensure client information is entered timely and accurately in the PRM database.
• Lead or participate in devotions, prayers, or other organizational activities that promote a credible Christian witness and lifestyle.

Requirements

• Bachelor’s degree in Social Work, Sociology, Psychology, Counseling, or another social service field, or 4 years of experience in case management, social work, or a similar setting, or a combination of education and experience.
• Certified in Peer Support (preferred).
• Current valid Arizona driver’s license with a clean driving record of 2+ years (verified by company MVR check) and ability to qualify for company insurance policy.
• Knowledge of the problems facing the street homeless population and awareness of resources that help people exit homelessness.
• High level of composure and ability to remain calm in stressful situations.
• Discretion and understanding of the importance of keeping client information confidential.
• Excellent communication skills.
• Proficiency in Microsoft Office apps.
• A solid and credible Christian witness and lifestyle that exemplifies Christ in character and actions.

Pay

$18.00 - $25.00/hour
