Responsibilities
- Maintain ongoing support and regular communication with existing and new or potential clients and referral sources through appropriate phone and face-to-face contacts (frequency defined by territory: number of face-to-face contacts per week, phone contacts per week, and new referral sources per month).
- Act as the primary representative and liaison between VIA Health Partners and clients, referral sources, and community education within the Education and Resource Manager’s (ERM’s) territory.
- Plan daily, weekly, and monthly activities and goals based on reports and feedback regarding top, medium, and low referral sources, client concerns, and other pertinent information.
- Ensure patients referred by ERM sources receive the highest quality of care; address any concerns promptly in accordance with VIA Health Partners policies.
- Collaborate closely with the Referral/Intake team and patient care teams to communicate information that exceeds client and referral source expectations.
- Assist in developing departmental strategies to strengthen relationships with existing clients and referral sources.
- Provide regular information sessions, in-services, and education to clients and referral sources on hospice, palliative care, and end-of-life care.
- Coordinate educational opportunities within the ERM’s region.
- Analyze data and develop strategic plans based on organizational needs.
- Attend community-based meetings and events (e.g., Chamber of Commerce, senior resource groups, marketing events at referral sources, community resource meetings, VIA Health Partners organizational and fundraising events) within the ERM’s territory to increase referral sources.
- Monitor competitive environment and regional communications; provide feedback to VIA Health Partners on the regional approach of other service providers.
- Participate in contract relationships between facilities and VIA Health Partners, serving as a liaison between the facility and Senior Director of Clinical Services and Vice President of Finance and Operations during contract negotiations.
Qualifications
- A Bachelor’s Degree in a related field is required.
- A minimum of 2 years of healthcare sales experience within the counties served by VIA Health Partners is required; hospice experience is preferred.
- Proficiency in Microsoft Office is required.
