Responsibilities
- Coordinate outreach efforts for the SSVF Program including establishing an outreach plan, developing a schedule, and communicating with the VA and other providers.
- Screen potential program participants for eligibility and verify information from VA-approved forms and applicable agencies.
- Conduct housing and comprehensive supportive services assessments and develop a Family Services Plan.
- Provide service coordination between the program and community partners, and assist in revising service plans and developing placement and follow-up plans.
- Support persons served and their families by providing or arranging for housing support and wraparound services, connecting to VA and mainstream resources.
- Travel to housing sites to support participants, monitor progress, and address health, behavioral, or life skills issues.
- Communicate with clients and service teams to address issues and adjust services as needed.
- Ensure appropriate use of and documentation of temporary financial assistance.
- Ensure participant category reassessments are completed according to specific SSVF requirements.
- Participate in program and professional supervision, attend meetings, and contribute to team-based collaborative planning.
- Complete all appropriate reporting in HMIS and other systems as required.
Qualifications
- A bachelor’s degree in counseling psychology, rehabilitative counseling, or a related human service field (or comparable experience).
- Experience working with homeless individuals and families, including veterans, is preferred.
