SSVF Carolinas Case Manager (68098)

About the role

Provide day-to-day support and services for participating veteran households. The position requires relevant experience in homelessness prevention, rapid re-housing, and transitional supportive services. You will provide key linkages for clients from intake and assessment through to attainment or maintenance of permanent housing, promoting accessibility, outreach, and wraparound supports in collaboration with the VA and community partners.

Responsibilities

  • Coordinate outreach efforts for the SSVF Program, including:
    • Establish an outreach plan targeting the three participant categories outlined in SSVF program guidelines.
    • Develop a set schedule to visit the VA, shelters, and other locations where homeless and low-income veteran families congregate.
    • Develop communication and updates with the VA and other key housing and community providers.
  • Screen potential program participants for eligibility using VA-approved forms and verify veteran and income status with applicable agencies.
  • Conduct housing and comprehensive supportive services assessments for program participants and develop a Family Services Plan.
  • Provide service coordination between the program and community partners for persons served and their families/supporters.
  • Revise service plans and develop placement, stabilization, and follow-up plans in collaboration with clients, family members, and agencies.
  • Provide or arrange housing support and wraparound services, including connections to eligible VA and mainstream resources.
  • Travel to housing sites to support clients, monitor progress, and address health, behavioral, or life skills issues.
  • Communicate effectively with the client’s service team to address and correct issues related to living skills, employment, education, family, and socialization.
  • Ensure appropriate use and documentation of temporary financial assistance.
  • Support veterans and families in securing rapid re-housing, financial independence, and community relationships.
  • Complete participant category reassessments according to SSVF requirements.
  • Participate in program and professional supervision, staff meetings, and trainings; contribute to team-based planning.
  • Complete all required reporting in HMIS and other systems.
  • Perform other duties as needed and requested.

Qualifications

  • A bachelor’s degree in counseling psychology, rehabilitative counseling, or a related human services field is preferred (or comparable experience).
  • Experience working with homeless individuals and families, including veterans, is a plus.

Requirements

  • Must be privileged to drive according to Agency MVR guidelines.
  • Valid driver’s license in jurisdiction of residence.
  • Vehicle liability coverage if driving a personal vehicle (may occasionally transport clients).
  • May be required to drive a van.
  • Acceptable criminal background check and OIG Exclusion check.
  • Negative drug screening.
  • CPR/First Aid Certification must be acquired and maintained once employed.

Physical Requirements

  • Ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies.
  • Physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, and hear.
  • Ability to perform basic and light home maintenance activities and operate office equipment requiring continuous or repetitive hand/arm movements.
  • Ability to remain in a sitting position for extended periods.
