Responsibilities
- Coordinate outreach efforts for the SSVF Program including establishing an outreach plan, developing a schedule, and communicating with the VA and other providers.
- Screen potential program participants for eligibility and verify information from VA-approved forms and applicable agencies.
- Conduct housing and comprehensive supportive services assessments, develop a Family Services Plan, and coordinate services with community partners.
- Provide or arrange for housing support and wraparound services, connecting clients to VA and mainstream resources.
- Travel to housing sites to support clients, monitor progress, and address health, behavioral, and life skills issues.
- Communicate effectively with clients, service teams, and other stakeholders to address issues and adjust services as needed.
- Ensure appropriate use of and documentation of temporary financial assistance and participant category reassessments.
- Participate in program and professional supervision, attend meetings, and contribute to team-based collaborative planning.
- Complete required reporting in HMIS and other systems.
Qualifications
- A bachelor’s degree in counseling psychology, rehabilitative counseling, or a related human service field (or comparable experience).
- Experience working with homeless individuals and families, including veterans, is preferred.
