About the role

We are seeking a Sr. STD Case Manager to join our Customer Success – Support team. This position is full-time and remote.

Responsibilities

  • Provide comprehensive case management for clients with Specialized Treatment Diversion (STD) programs.
  • Collaborate with multidisciplinary teams to develop and implement care plans that meet client needs.
  • Ensure timely and accurate documentation of all client interactions and progress notes.
  • Conduct regular check-ins with clients to monitor their progress and address any issues promptly.
  • Participate in ongoing training and professional development opportunities to enhance skills and knowledge.

Requirements

  • Bachelor's degree in a related field (e.g., social work, public health, psychology).
  • At least 3 years of experience in case management or a similar role.
  • Experience working with individuals in STD programs or similar high-risk populations.
  • Strong communication and interpersonal skills.
  • Proficiency in Microsoft Office Suite.

Qualifications

  • Knowledge of mental health and substance abuse treatment models.
  • Ability to maintain confidentiality and handle sensitive information.
  • Experience with data entry and record-keeping systems.
