About the role
We are seeking a Sr. STD Case Manager to join our Customer Success – Support team. This position is full-time and remote.
Responsibilities
- Provide comprehensive case management for clients with Specialized Treatment Diversion (STD) programs.
- Collaborate with multidisciplinary teams to develop and implement care plans that meet client needs.
- Ensure timely and accurate documentation of all client interactions and progress notes.
- Conduct regular check-ins with clients to monitor their progress and address any issues promptly.
- Participate in ongoing training and professional development opportunities to enhance skills and knowledge.
Requirements
- Bachelor's degree in a related field (e.g., social work, public health, psychology).
- At least 3 years of experience in case management or a similar role.
- Experience working with individuals in STD programs or similar high-risk populations.
- Strong communication and interpersonal skills.
- Proficiency in Microsoft Office Suite.
Qualifications
- Knowledge of mental health and substance abuse treatment models.
- Ability to maintain confidentiality and handle sensitive information.
- Experience with data entry and record-keeping systems.
