Speech Therapist ST/ ST CFY

This position is for a skilled nursing facility with both short-term and long-term patients. Harbor Point Skilled Nursing has an in-house therapy team with smaller daily case loads, all new equipment, and an emphasis on optimizing time with patients. Responsibilities

• Develop and implement a Speech Therapy plan of care for facility residents and short-term patients
• Assist as needed with patient screens for Speech Therapy
• Communicate progress and any barriers toward SLP goals with interdisciplinary team members
• Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements
• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods
• Provide and document patient, family, and caregiver training regarding each speech therapy patient's specific plan of care and recommendations for follow-up
• Maintain productivity standards as expected by the Director of Rehab or Administrator

• Current Texas licensure as an SLP is required
