About the role
The Speech-Language Pathologist (SLP) role at Adaptive Home Health is central to improving patient access to home health care. You are responsible for restoring communication, cognition, and swallow safety, ensuring patients can function safely at home. Additionally, you build caregiver carryover to sustain progress between visits.
Responsibilities
- Own patient outcomes: develop and execute a clear plan of care with measurable, functional goals.
- Complete comprehensive in-home SLP evaluations and provide skilled visits.
- Assess and treat communication disorders, cognitive-communication impairments, and dysphagia.
- Drive multidisciplinary coordination by communicating changes in condition and aligning on orders.
- Coach patients and caregivers with practical strategies and routines.
- Document promptly and thoroughly in Homecare Homebase (HCHB).
Requirements
- Active Texas SLP license (in good standing)
- Strong clinical reasoning and ability to operate independently in the field
- Comfort identifying red flags and escalating changes in condition to RN/MD
- Strong caregiver education skills (teach-back, carryover planning)
- Strong communication and documentation skills
- Reliable follow-through on orders, frequencies, and patient goals
- Current driver’s license and comfort with daily travel across assigned territory
