Speech Therapist (ST) Home Health

Responsibilities

• Provide services within the scope of speech therapy, in accordance with the plan of care individualized for the patient, and in coordination with the Interdisciplinary team
• Follow the plan of care; provides treatment; evaluate patient's reaction to treatment and report any changes in his/her condition to the Physician, Case Manager and/or or Director of Nursing
• Maintain appropriate clinical records and/or notes and submit the records with organization policy
• Be responsible for following all company policies and procedures

Requirements

• Current ST License to practice in California
• Home Health: 1 year - Preferred
• Current CA Driver's License
• Coverage area: Santa Clara County

Pay

$60.00 - $75.00 per hour (depending on experience)
