Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, is seeking a PRN Speech Language Pathologist (SLP) to deliver compassionate, patient-centered care in Volusia and Flagler Counties.

Responsibilities

  • Evaluate clients’ speech, language, and hearing abilities, recommending strategies to enhance communication and hearing.
  • Conduct initial and ongoing comprehensive assessments, including Outcome and Assessment Information Set (OASIS) evaluations.
  • Develop and revise treatment plans in consultation with physicians as necessary.
  • Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed.
  • Educate clients and families on speech therapy techniques and related challenges.
  • Document all evaluation data, treatments, and client progress promptly.
  • Supervise Home Health Aides every 14 days when Speech Therapy is the sole discipline involved.
  • Communicate regularly with physicians, ensuring orders are appropriate and addressing any changes in client condition.
  • Participate in the periodic revision of care plans, updating the physician on client status and obtaining additional orders as required.

Requirements

  • Graduate of an accredited Speech Therapy program.
  • Licensed Speech Therapist in Florida.
  • Minimum of two years of clinical experience as a Speech Therapist.
  • Current BLS Certification.
  • Valid driver’s license and insured, reliable vehicle.
  • Strong emotional resilience and physical stamina, including the ability to stand, stoop, and bend frequently.
  • Excellent judgment, communication, and observational skills.
  • Self-motivated with the ability to work independently.
