Schedule
Monday - Friday with on call weekends rotation
Responsibilities
- Evaluate patients’ speech, language, communication, and swallowing-related needs, including assessing the home environment and identifying appropriate equipment.
- Develop, implement, and update individualized speech therapy treatment plans and Plans of Care (POC) in collaboration with the physician and care team.
- Provide skilled speech therapy services using evidence-based therapeutic techniques and specialized equipment to improve functional outcomes and quality of life.
- Monitor patient progress and communicate changes in condition, treatment response, and functional status with the physician, case manager, and interdisciplinary care team.
- Complete accurate, timely, and thorough clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.
- Educate patients, caregivers, and other healthcare personnel on appropriate therapeutic strategies, communication techniques, and ongoing care needs.
Requirements
- Current state licensure as a Speech-Language Pathologist
- Master’s or Doctoral degree in Speech-Language Pathology
- Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting
- Current CPR/BLS certification
- Valid driver’s license and insured personal vehicle (per Lorian requirements)
Pay
This position is based on a pay per visit compensation model. Pay per visit ranges from $120-$125 depending on type of visit and experience.
