About the Role

The Speech Therapist provides assessment, evaluation, and treatment for patients with speech, language, swallowing, and cognitive disorders in a home health setting.

Responsibilities

• Conducts diagnostic and functional assessments to determine patient needs.
• Develops and implements individualized treatment plans in collaboration with the healthcare team.
• Performs therapeutic interventions to improve communication, swallowing, and cognitive functions.
• Monitors and documents patient progress, reporting responses and changes to supervising staff.
• Educates patients, families, and caregivers on therapy techniques and supportive strategies.

Requirements

• Licensed in Speech Therapy within the respective state.
• Current CPR certification.
• Valid driver’s license, reliable vehicle, or access to transportation.
