Speech Therapist

Role: Speech Therapist providing personalized care in patients' homes across Western Colorado. Responsibilities: Assess speech, language, cognitive-communication, and swallowing disorders;Develop and implement individualized treatment plans;Collaborate with an interdisciplinary team to coordinate care;Educate patients and families on treatment goals and strategies;Document progress and adjust plans as needed. Qualifications & Skills: Strong clinical skills in speech therapy, excellent communication abilities, ability to work independently, and a patient-focused approach. Experience in home health preferred. Key Duties: Provide specialized therapy, support patient independence, foster strong patient relationships, and contribute to a collaborative, community-centered environment.
