About the Role

Provides comprehensive speech therapy services in home settings, including evaluating patients, developing individualized treatment plans, and delivering therapy to address speech, language, cognition, voice, and dysphagia deficits.

Responsibilities

  • Assess patients using diagnostic tools to determine functional levels and intervention needs.
  • Develop and modify personalized treatment programs based on ongoing evaluations.
  • Deliver direct therapy tailored to patients' physical, emotional, and cognitive abilities.
  • Coordinate therapy within multidisciplinary teams, ensuring alignment with overall care plans.
  • Document evaluation results, progress notes, and treatment plans accurately.
  • Communicate effectively with patients, families, and healthcare team members.
  • Participate in program development and supervise clinical internships.

Requirements

  • CCC-SLP from ASHA or active supervised clinical fellowship with certification within one year.
  • Active state license as a Speech-Language Pathologist.
  • Preferred 1-3 years of experience in hospital, outpatient, SNF, or home health settings.
  • Effective communication, teamwork, and adherence to safety and confidentiality standards.
