About the role
Provide evaluation, treatment, and therapy services in a home setting.
Responsibilities
- Conduct assessments and develop individualized treatment plans.
- Implement therapy strategies and modify interventions based on ongoing evaluations.
- Communicate effectively with patients, families, and interdisciplinary team members.
- Prepare documentation including evaluation results, progress notes, and treatment reports.
- Participate in program development and supervise clinical internships.
Requirements
- CCC-SLP certification from ASHA, or active supervised clinical fellowship, or active state license.
- 1–3 years of experience in hospital, SNF, outpatient, or home health settings preferred.
