About the Role
Provides speech therapy services to patients with speech, language, or swallowing disorders, ensuring personalized care within a home health or hospice setting.
Responsibilities
- Assess and document patient speech, language, and swallowing data, tracking progress and outcomes.
- Review and modify treatment plans based on individual performance and changing needs.
- Instruct patients on monitoring speech and practicing new skills.
- Maintain accurate documentation in compliance with legal, policy, and regulatory standards.
- Identify trends in physical and emotional parameters affecting therapy.
- Perform additional job-related duties as assigned.
Requirements
- Master's Degree or foreign equivalent in Speech-Language Pathology.
- State licensure as a Speech Pathologist.
- CPR or BLS certification from an approved provider.
- Valid Driver's License and minimum age of 21.
Preferred: ASHA Certificate of Clinical Competence (CCC).
Organizational Requirements
