Speech Language Pathologist (Sign-On Bonus)

Responsibilities

• Deliver one-on-one speech therapy to patients in private residences and senior communities.
• Perform evaluations, develop personalized care plans, and provide evidence-based treatment.
• Support patient wellness, mobility, and independence through therapy sessions.
• Document care effectively within the EMR system, adhering to regulatory standards.
• Collaborate with interdisciplinary teams including PTs, OTs, and fitness specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Utilize telehealth and virtual care options to increase access and flexibility.

Qualifications & Skills

• Valid state licensure or eligibility for licensure as an SLP.
• Degree from an accredited Speech Language Pathology program.
• Proficiency with computers and EMR systems.
• Current CPR certification.

Physical Demands

Ability to perform moderate physical effort, assist with patient transfers, reposition patients, and drive between patient locations.
