Speech Language Pathologist (Sign-on Bonus)

About the role

Provide personalized speech-language pathology care to older adults in private homes and senior living communities, with options for in-person and virtual therapy.

Responsibilities

• Conduct evaluations and develop individualized care plans
• Deliver evidence-based therapy to enhance mobility and independence
• Document patient progress in electronic medical records (EMR)
• Collaborate with interdisciplinary care teams
• Educate patients and caregivers on therapy techniques and goals
• Utilize telehealth platforms when appropriate

Key Duties

• Provide one-on-one therapy sessions
• Support overall patient wellness and aging in place
• Promote community-based care initiatives
• Flexible treatment scheduling and virtual care options

Requirements

• Valid SLP license or eligibility for licensure
• Degree from an accredited speech-language pathology program
• ASHA Certificate of Clinical Competence (CCC)
• Current CPR certification
• Proficiency with EMR systems and computer-based documentation
• Physical ability to perform in-home care tasks
