About the role
Speech Language Pathologist responsible for delivering personalized in-home and senior living care to older adults, utilizing a flexible, patient-centered approach.
Responsibilities
- Provide one-on-one speech therapy in private residences and senior communities
- Perform evaluations, develop individualized treatment plans, and deliver evidence-based interventions
- Support patient wellness, mobility, and independence through therapy sessions
- Document care activities accurately in electronic medical records
- Collaborate with interdisciplinary teams including PTs, OTs, and fitness specialists
- Educate patients, caregivers, and staff to promote safety and aging in place
- Utilize telehealth and virtual care to extend access and flexibility
Qualifications & Skills
- Valid state license or eligibility in Speech Language Pathology
- Degree from an accredited program and current CCC from ASHA
- Proficiency in basic computer and EMR systems
- Current CPR certification
Physical & Other Requirements
- Ability to perform moderate physical effort, assist with patient mobility, and drive between locations
- Must wear PPE as needed
