Speech Language Pathologist (Sign on Bonus!)

About the Role

Seeking licensed Speech Language Pathologists to provide personalized, one-on-one care to older adults in private homes and senior living communities. The role involves performing evaluations, developing care plans, delivering evidence-based therapy, and documenting within electronic medical records.

Responsibilities

• Conduct assessments and deliver therapy to support patient independence and mobility.
• Collaborate with interdisciplinary teams including PTs, OTs, and staff.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Utilize telehealth and virtual care to enhance service delivery.

Requirements

• Valid SLP license and Certificate of Clinical Competence (CCC) from ASHA.
• Degree from an accredited Speech Language Pathology program.
• Proficiency in electronic medical records (EMR) and basic computer skills.
• Current CPR certification.

Physical & Additional Requirements

• Ability to perform moderate physical tasks, including lifting, repositioning, and assisting with transfers.
• Ability to drive and wear PPE as needed.
