How You'll Work

• You'll provide home-based skilled speech therapy care for patients as directed by the attending physician.
• You will educate patients and their family members and ensure the safety of the patient.

Major Areas of Responsibility

• Plan of Care: Develop an individualized ST plan of care and make adjustments as needs change.
• Guide the patient: Instruct the patient in therapeutic, creative, and self-care activities to improve functioning.
• Observe, record, and report: Monitor patient reactions, changes, and progress and report to the attending physician and any appropriate agency staff.
• Minimize Patient Risk: Teach the patient on preventative measures and make good decisions to reduce re-hospitalizations.
• Family Educator/Advocate: Counsel the patient and family on the disease/injury and how to manage throughout all phases of speech therapy.
• Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
• Operations: Attend in-service training and mandatory agency meetings.

Requirements

• Speech Therapist with a current license in the state of employment
• Minimum one-year experience as a ST in an acute care setting
• Home care experience preferred
• Valid driver's license and auto insurance in your name as a driver
• Capable of all physical demands
