How You'll Work
- You'll provide home-based skilled speech therapy care for patients as directed by the attending physician.
- You will educate patients and their family members and ensure the safety of the patient.
Major Areas of Responsibility
- Plan of Care: Develop an individualized ST plan of care and make adjustments as needs change.
- Guide the patient: Instruct the patient in therapeutic, creative, and self-care activities to improve functioning.
- Observe, record, and report: Monitor patient reactions, changes, and progress and report to the attending physician and any appropriate agency staff.
- Minimize Patient Risk: Teach the patient on preventative measures and make good decisions to reduce re-hospitalizations.
- Family Educator/Advocate: Counsel the patient and family on the disease/injury and how to manage throughout all phases of speech therapy.
- Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
- Operations: Attend in-service training and mandatory agency meetings.
HARD & SOFT SKILLS

- Attention to detail is critical, as is being observant and following directions
Requirements
- Speech Therapist with a current license in the state of employment
- Minimum one-year experience as a ST in an acute care setting
- Home care experience preferred
- Valid driver's license and auto insurance in your name as a driver
- Capable of all physical demands
