About the Role
Join the transplant department as a Social Worker, providing specialized services to individuals with alcohol-related liver disease. This role involves facilitating group and individual therapy for patients pre- and post-transplant, conducting psychosocial assessments to identify barriers for transplant candidacy, and supporting patients and their families in coping with physical, emotional, and environmental challenges.
Responsibilities
- Conduct psychosocial assessments and reassessments of patients and significant others to evaluate social, emotional, and cognitive capabilities in relation to illness and health maintenance needs.
- Facilitate group and individual counseling for patients with alcohol-related liver disease.
- Develop and implement intervention plans addressing needs identified during assessments.
- Collaborate with the multidisciplinary healthcare team to establish and ensure appropriate discharge planning.
- Provide counseling to patients, families, and significant others to assist with coping related to new diagnoses, chronic illness, palliative care, or terminal care.
- Advocate on behalf of patients and families to help them obtain necessary services.
- Interact with co-workers, visitors, and staff in alignment with Jefferson’s values.
Requirements
- Graduate from an accredited Master of Social Work (MSW) program.
- Minimum of 2 years of hospital-based social work experience.
- Experience with group therapy, individual therapy, and working with individuals affected by drug and alcohol use.
- Active LSW or LCSW licensure for Pennsylvania positions.
