Social Worker - PT

About the Role

The Social Worker is responsible for providing services to terminally ill patients in their residences under physician orders. The Social Worker assists in meeting the psychosocial needs of the patient and their caregivers, enabling patients to achieve an optimal level of comfort. Performs duties and responsibilities in a manner consistent with Mercy's mission, values, and service standards.

Part-Time (20 hours per week)

Qualifications

• Bachelor’s degree in Social Work required.
• Minimum of one year’s experience in a healthcare setting; hospice experience preferred.
• Experience with issues related to bereavement and terminally ill patients preferred.
• Good oral and written communication skills.
• Good organizational skills.
• Must be a licensed driver.
• Ability to work as a member of an interdisciplinary team.

Skills

• Basic computer skills, including working knowledge of Windows, Word, and Outlook (preferred).
