About the role
Launch and lead a new palliative care program, overseeing all aspects of startup, operations, and growth.
Responsibilities
- Oversee startup activities including workflows, policies, and operations
- Manage program budgets
- Supervise staff and ensure regulatory compliance
- Support provider credentialing and payer contracting
- Facilitate referrals, documentation, and care transitions
- Drive program growth and quality improvement initiatives
- Deliver social work services as needed
- Participate in on-call rotations
Requirements
- Master’s degree in social work (required)
- Hospice or palliative care leadership and program development experience (preferred)
- Proficiency with Electronic Medical Records and Microsoft Office
- Strong communication, leadership, and interdisciplinary collaboration skills
