Social Worker- Palliative Care Manager

You’ll join a mission‑driven team that values courage, compassion, and creativity while helping launch and grow a brand‑new palliative care program from the ground up.

Key Responsibilities

  • Lead all program startup activities, including workflows, policies, and operational design
  • Manage program budget and financial performance
  • Supervise and support palliative care staff in collaboration with the Medical Director
  • Oversee compliance with Medicare, Medicaid, commercial payers, and accreditation standards
  • Support provider credentialing and payer contracting
  • Ensure efficient scheduling, referrals, documentation, and care transitions
  • Drive program growth, customer service, and performance improvement
  • Deliver social work services during start up and as needed
  • Participate in the Manager On-Call Rotation for hospice services

Qualifications

  • Master's degree in social work required
  • Hospice or palliative care related leadership experience preferred
  • Prior program development experience preferred
  • Experience in Electronic Medical Records and Microsoft Office products required
  • Strong interpersonal, communication, and leadership skills with a demonstrated ability to collaborate effectively across disciplines and guide teams through periods of growth and change
