Social Worker, Norton Children's Medical Group Cardiology

Responsibilities

• Member of the multidisciplinary team, collaborating directly with the care team, care management team, patient, family, and/or patient’s representative to assess the patient’s continuum of care needs.
• Ensures patients transition to the least restrictive care environment.
• Assesses the patient’s psychosocial and continuum of care needs as an independent professional.
• Provides interventions including, but not limited to:
  • Family support
  • Crisis management
  • Consultations with staff physicians
  • Adult or Child Protective Services reporting/coordination
  • Custody/guardianship issue coordination
  • Indigent patient program coordination
  • Substance-use support coordination
  • Intimate partner violence support
  • Bereavement support
• Coordinates referrals and resources such as acute rehabilitation, extended care, durable medical equipment, home health, and other necessary services.
• Adheres to all Norton Healthcare policies and applies educational and professional expertise in delivering quality care.

Qualifications

• Required:
  • One year of social work experience or one year of social work internship.
  • Hospital and/or payer credentialing completed before functioning in this role.
  • Master’s Degree in Social Work from an accredited program.
  • Kentucky - Certified Social Worker (CSW) or Indiana - Licensed Social Worker (LSW).
• Desired:
  • One year in a healthcare setting.
  • Licensed Clinical Social Worker (LCSW).
