About the role

The Inpatient Master of Social Work (MSW) Care Manager provides professional, comprehensive patient centric care management services for at-risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy.

Responsibilities

• Assessment and planning of patient needs
• Coordination of interdisciplinary care teams
• Discharge planning and follow-up services
• Documentation of interventions and care plans
• Ensuring regulatory compliance and adherence to standards
• Patient advocacy and support

Requirements

• Master's Degree in Social Work from a CSWE-accredited program
• One year of SW experience in any healthcare setting
• One year of experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program

Qualifications

• Alaska Masters Social Worker License or Temporary License
• National Military Courtesy License

Skills

• Basic understanding of acute medical surgical healthcare terminology
• Experience in acute care settings

• Base pay range: $35.32 - $54.82
