Social Worker MSW - Per Diem - Varied - 8hr FPH

Job Summary

Counsels patients on their current and future concerns through psycho-social assessments.

Evaluates patients' past living conditions in relation to their current medical status to create effective discharge plans.

Reduces patient anxiety about their health and assists in arranging post-hospital care referrals.

Collaborates with the healthcare team to improve the social, emotional, and psychological care provided to patients, families, and significant others.

Able to travel between multiple work sites as needed.

Job Requirements

• Minimum Education: MSW from an accredited school of Social Work.
• Experience: At least 1 year of experience working in a hospital setting is preferred.
• Skills: Excellent customer service skills are required.
• Licenses: BLS certification is mandatory.

Pay

$43.00 - $43.00 per hour
