About the role

The Social Worker at Marian Regional Medical Center will provide vital support and guidance to individuals and families facing complex challenges. Responsibilities include conducting psychosocial assessments, developing care plans, providing counseling and crisis intervention, and connecting clients to resources. The role also involves advocating for clients and collaborating with multidisciplinary teams to ensure holistic support and promote well-being.

Responsibilities

• Conduct psychosocial evaluations of patients and families, including a detailed psychosocial history.
• Assist in evaluating environmental and psychological implications of illness, formulate a social casework diagnosis, and develop plan for social service interventions and goals in collaboration with the patient and/or family and physician.
• Provide direct casework services for patients and families, implementing physician-ordered social service interventions.
• Make referrals to appropriate community agencies and provide follow-up to assure patients progress.
• Make referrals to appropriate agencies in cases of suspected child/elder abuse.
• Provide crisis intervention to patients and staff.

Requirements

• Masters in Social Work and one year social work experience in a healthcare setting, upon hire.
• Basic Life Support - CPR, within 30 Days.
• Driver's License: CA, upon hire.
• Auto Insurance, upon hire.

Preferred

• Home Health experience.

Pay

The posted compensation range of $46.12 - $54.83 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
