Key Responsibilities

• Provide emotional and practical support to patients and their families in their homes
• Assess patients' needs and develop individualized care plans
• Link patients with community resources and services
• Collaborate with interdisciplinary healthcare teams to provide holistic care
• Maintain accurate and up-to-date patient records
• Stay current with advancements in home health care and social work practices

Requirements

• Must have Bachelor's or Master's degree in Social Work
• At least one year of Medical Social Work experience; home health experience preferred
• Experience in health care, home care or home health care settings
• Ability to work independently and manage caseload
• Strong understanding of health care systems and policies
• Proficiency in electronic medical records (EMR) systems
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines
• Ability to effectively utilize EMR and other technology to maintain HIPAA privacy, continuity of care, and team communication

• Strong critical thinking skills when working independently in the field
• Excellent interpersonal skills to build rapport with diverse patient populations
• High level of organization and time management to navigate a daily route
