Home Health MSW Social Worker

About the role

Haven Home Health & Hospice is a locally owned and clinician-led organization that believes in providing high-quality patient care. The ideal candidate will have a Master’s Degree in Social Work and active Missouri licensure, at least one year of MSW experience within the last three years, and experience in home health or hospice settings. Experience with geriatric populations is preferred.

Responsibilities

• Develop and implement individualized MSW plans of care in collaboration with physicians and the interdisciplinary team
• Absess psychosocial needs and determine eligibility for home health services
• Provide counseling, resource coordination, and crisis intervention
• Re-evaluate and adjust care plans as patient needs change
• Support patients and families navigating complex healthcare decisions

Requirements

• Master’s Degree in Social Work and active Missouri licensure required
• Minimum one year MSW experience within the last three years
• Home health or hospice experience preferred
• Experience with geriatric population preferred
• Flexibility to meet patient scheduling needs, including new admissions
