Responsibilities

• Provide social work services to patients and families in home health settings
• Collaborate with interdisciplinary teams to develop and implement care plans
• Assess patient needs and provide counseling and support
• Ensure compliance with healthcare regulations and standards

Requirements

• MSW degree from an accredited school of social work (CSWE)
• One year of experience in Home Health Care (preferred)
• Valid Driver's License, car insurance, and reliable transportation
